LIVERMORE FALLS — Jessica Hopkins will join the medical team at Western Maine Family Health Center in April.

Hopkins is a licensed clinical social worker who has been implementing a new model of integrated behavioral health into health services at another practice, Belgrade Regional Health Center, since last year. Now she will also be serving patients and families who obtain health care at Western Maine.

One of the program goals is to focus on the total person and assist patients at difficult crossroads across the lifecycle. The Western Maine medical providers will recommend patients to Hopkins and she will meet with them right in the health center. This will include patients who would benefit from working with a trained professional in handling a new diagnosis, managing a chronic condition, setting limits with children, and many other factors that influence people’s overall wellness.

Hopkins earned a master in social work degree from the University of New England and a bachelor of arts degree in communication from the University of Southern Maine. She brings experience with infants, children and families in a variety of settings, including neonatal intensive care, residential, outpatient and outreach environments.

Hopkins joins Dr. Craig Lewis and family nurse practitioners, Emma Ansara and CeCe Rohrbach, who provide medical care to more than 2,500 residents and visitors from Canton, Fayette, Hartford, Jay, Livermore, Livermore Falls, Sumner, Chesterville, Farmington, New Sharon, Wilton and surrounding communities.

Western Maine Family Health Center is a practice of HealthReach Community Health Centers, a system of 11 federally qualified health centers in Central and Western Maine. A private, nonprofit organization celebrating 35 years of providing care, HealthReach is funded by patient fees, grants and individual donations. Visit www.HealthReachCHC.org.
